Adobe Reader is free software you can download from the Internet. It will be beneficial for you to have this installed in your computer, as many other web sites have info in the Adobe format.

To Install

1. Click on the "Download Adobe Acrobat Reader Free" on this web site.
2. Scroll down to the bottom of the page and click the "Get Acrobat Reader Free". It is the second choice in red.
3. Complete step 1. Be suer to specify your operating system. Then click the "Download" button under step 3.
4. Choose the option "Save this Program to Disk".
5. When it asks where to save, indicate Save In "Desktop", then click "Save". Be sure that the "Close this dialog box when download completes" box is NOT checked.
6. When download is complete, click "Open".
7. You'll go thru a "wizard" setup, click "next" and "next" again.

You have now successfully installed Adobe Acrobat on your computer. Click the small "x" in the upper right-hand corner of their web site, and it will bring you back to our web site.

When you click on the New Patient Form, you'll receive a message from Adobe, click "Accept" on their agreement, and our form will appear.
