For your initial visit, we request that you are present at least 15 minutes prior to your appointment time. Please bring your insurance card.

If canceling an appointment, we respectfully request a 24 hour notice.

Insurance

We accept a wide variety of insurance plans and policies.

Medicare and Medicaid patients are welcome. Medicare patients may be requested to pay for initial services at the time of the visit, so that Medicare reimbursement can be sent directly to your home.

We will try to advise patients in advance if treatment required is not a covered benefit under insurance policies.

Payment plans can be employed.
